Recently there has been a big shift away from silver fillings and crowns with metal in them due to a fear of caustic metals such as mercury in the alloys. At Enamel we use biocompatible materials such as composite resins or Porcelain. When the decay in the tooth is small we can use composite resins, commonly the material for white fillings, to restore the tooth. As the cavity gets bigger and more tooth structure is lost porcelain must be used to restore the tooth. If the cavity is medium sized an inlay or onlay can be done which is like a porcelain filling. This allows to be as conservative as possible. Lastly if there was a large cavity, many times a crown is needed. The crowns at Enamel are made out of porcelain and they cover and protect the tooth. Composites and porcelain are more biocompatible with the body and don’t leak any heavy metals or irritate any tissue. Replacing old metal fillings and crowns
Many people have old Metal fillings which are leaking and causing their teeth to look grey. When we see this during our exam we can recommend what to replace the metal with. Sometimes it’s as simple as a Bis-GMA free filling or sometimes more extensive work is needed because of the leakage from the filling. If we need to reinforce the tooth more we can choose from an Inlay/onlay or an all porcelain crown. Porcelains crowns and porcelain inlays/onlays are stronger than the filling material because it is processed in the lab. As your cosmetic dentist, whatever your need is we can make that tooth look white and beautiful again.
We also have many patients that have old crowns where you can see the grey by the gum line. Lots of people don’t know what the grey is. It can be decay leaking under the crown, the metal in the substructure of the crown, or the tooth that has darkened. At Enamel Newport Beach we have some tricks to take that old metal substructure crown off and mask the tooth so its white and place an all porcelain crown. Bis-GMA free composite fillings
In 1986 Proposition 65 was passed which required businesses to inform their clients/patients if that was anything possibly unsafe in the materials they were using. For dentists this meant that they had to warn patients about the mercury in amalgam (silver) fillings. Recently, we have discovered that some composite filling materials contain Bis-GMA which can be harmful to us. As your holistic dentist we strive to use the safest materials and maintain the cleanest dental office possible. Having said that, when a patient comes in with small cavities or small silver fillings they want to replace we use Bis-GMA free composite fillings to make teeth whole again. We have different shades to match the color of your teeth so it looks like the tooth was never touched to begin with!
Inlays and Onlays
When tooth decay is medium sized or if we are replacing a medium sized silver filling we will opt to restore the tooth with a porcelain inlay or onlay. The advantage to using inlays and onlays are their strength. Because they are made out of porcelain in the lab they provide superior strength compared to fillings. As your dentist we will indicate when this treatment is needed. Inlays and onlays also hold their polish and hinder stain build up better than white fillings. The disadvantage to white fillings is that it takes multiple appointments to complete an Inlay or onlay similar to a porcelain crown. The first appointment your dentist will prepare the tooth and you will leave with a temporary filling in place. After 2 weeks the patient will return to the dental office and the dentist will cement the onlay or inlay to complete the procedure.
For many years dentists have been using metal crowns and metal crowns with porcelain layered over them. This gives the teeth an opaque look to them and you sometimes you can see the metal margin at the gum line. In the event the color match isn’t perfect we can even have the lab technician check out your smile to get the perfect color to make your smile seemless.
